Are you a good candidate for LASIK?

People who are most satisfied with the results of laser vision correction understand the criteria for eligibility, possess realistic expectations of what their vision will be like after surgery, and clearly understand the potential risks and side effects.

Not everyone can have refractive surgery. Eligibility depends on the amount of refractive error, the curvature and thickness of the cornea and a number of other factors that an ophthalmologist must evaluate.

The following questionnaire is designed to help you determine if you may be eligible for LASIK.

Has your lens prescription changed significantly within the last year?
Are you pregnant?
Are you afflicted with unstable or uncontrolled diabetes?
Do you suffer from an autoimmune disease?
Do you take drugs or undergo therapy that suppresses your immune system?
Have you been diagnosed with an uncontrolled vascular disease?
Do you have medical problems related to your eyes, such as amblyopia (lazy eye), keratoconus or glaucoma?  (This may not necessarily rule you out for LASIK, PRK or AK - but it might.)
Are you under 18 years of age?

The criteria that surgeons use to determine eligibility vary. The following questionnaire is designed to alert patients to conditions that surgeons will evaluate on a case-by case basis.

Are you nursing or expecting to become pregnant within the six months following the LASIK procedure?
Do you have a history of excessive scarring with injuries or other surgeries?
Do you suffer from chronic herpes infections or connective tissue disorders?
Do you have severe dry eye?
If you are myopic, do your pupils dilate to greater than 6.5mm in dim light?
If you are hyperopic, do your pupils dilate to greater than 9mm in dim light?
Do you have myopia greater than -14D and/or >5D of astigmatism?
Do you have hyperopia greater then +6D and/or >1D of astigmatism?

Not everyone who can have LASIK should. The following questionnaire is designed to assist in determining if you are a good candidate for the LASIK procedure.

Do you have a strong desire to be free of glasses or contact lenses?
Do your glasses or contacts interfere with your job, studies, sports, parenting or daily activities?
Do you clearly understand and accept the risks of surgery?
Do you clearly understand that the effects of LASIK are permanent and that the effects of LASIK do not wear off?
Do you understand that refractive procedures require follow-up examinations at specific intervals?
Do you have time to attend these examinations?
Are your expectations of refractive surgery realistic?

The following questions require more personal, more in-depth answers:

 
Do you know if you are you nearsighted, farsighted, presbyopic, astigmatic?
What is your current lens prescription?
Do you wear contacts or glasses or both or neither?
If glasses, do you only wear glasses for reading?
If contacts, soft lenses or gas permeable?
And what is your age?
How is your overall eye and general physical health?
If taking medication, what are you taking and what for?
Have you ever been told you ARE NOT a candidate for any vision correction procedure? If so, why?
How long have you been thinking about having LASIK? Longer than 6 months?
Have you had a chance to meet with other surgeons in your area?
If so, why didn't you proceed with LASIK at that time?
What is it that you are looking for, today? In other words, if you are a good candidate for LASIK, what are you looking to have answered - what do you still need - that will enable you to proceed with the procedure at this time? Financing? (We have great payment plans) Want to watch a procedure? Need spouses approval?.......
Additional comments or questions:

People who are not comfortable with the possibility that they may still need glasses or contact lenses - at least part-time after surgery - are probably not good candidates. You must carefully weigh your hoped-for benefits against the calculated risks. After all, no surgeon can guarantee 20/20 vision in every situation.
